Student Flu Vaccine Clinic
We are pleased to be able to offer your child the Flu Vaccine while at school this year on Tuesday, October 22, 2024 (Enfield and STMS) and Wednesday, October 23, 2024 (Erdenheim and STHS) during the school day. There will be no cost to families. Only the Flu Shot will be given at this clinic. Please read an important letter about the upcoming clinic.
Paper consent forms will be coming home with the students during the first week of school. Consent forms are also provided electronically for download below and, additionally, may be requested from the school nurse. Please complete and return your child's consent form to the school nurse no later than Wednesday, October 16, 2024. Please make sure that you review this paperwork and complete all required sections. Once the consent form is received, your child will be put on the schedule to receive their shot during the school day.
If you need to cancel your child's registration to receive the flu shot, please do so by contacting your child's school nurse via email at least 2 days prior to the scheduled clinic.
